Physical Exams~

- A physical examination is a routine test your primary care provider             (PCP) performs to check your overall health.

- A PCP may be a doctor, a nurse practitioner, or a physician assistant.

- The exam is also known as a wellness check.

- You don’t have to be sick to request an exam.

Types of Physical Exams

Annual Physical

An annual  physical examination helps your PCP to determine the general status of your health.

 

The exam also gives you a chance to talk to them about any ongoing pain or symptoms that you’re experiencing or any other health concerns that you might have.

 

A physical examination is recommended at least once a year, especially in people over the age of 50.

Any modification for any Medicare PX exam needs to be handle by Medicare Department only

Sports Physical

A sports physical, known in the medical field as a

pre-participation physical examination (PPE), helps to determine whether or not a child (or adult) is physically able to participate in a sport safely.

INS- Immigration Exam

An immigration medical exam is a necessary part of immigrating to the United States and becoming a permanent resident (green card holder).

 

Sometimes called a green card medical exam, the appointment is a routine part of the process to ensure public safety and remove the grounds for inadmissibility for intending immigrants.

Important information:
Fees/cost of the I-693 exam and/or immunizations are not covered by insurances and must be paid before any services are rendered.
The envelope with forms and results must be sealed with clinic stamp and cannot be opened by patient. Also cannot be given to another person other than the patient (unless the patient is a minor then it will be given to to the parents).
Result time frame is 14 business days (if everything is normal). The envelope pick up time is M-F 9am to 4pm
There is a separate rush fee of $60 if forms are needed before the 14-day time frame.
