Referral Protocol 

How to manage Referral Calls

It is very important we identify when does a call needs to be transferred to referrals and when we can provide
information yourself.
if you are transferring a caller, provide the Referral number
951-588-2653. In case the call drops they can call them directly.

Screening Steps

Who's calling 

and from where?

What is the referral for?

(which specialist)

Were you able to locate

the referral?

Can we help

them?

Is it imaging

related with the breast?

Is Pt Rios

Southwest?

DO NOT transfer to referrals if:
If a Reminder states we need to forward test to specialist. (if not
listed on chart we need to set appointment with PCP)

Patient wants to see a specialist and has not been referred by PCP(set
an appointment).

 
Patient has Direct Referral (Dentist, Optometrist) Inform caller they
can go where they prefer. (If a patient is Brand New Day/ Rios
Southwest they need to be transferred to the Medicare Department).

 
Patient is calling to request the specialist number or appointment that
patient was referred to. (see referral notes)

 
If the Referral status is "pending appointment" (Check Notes &
provide information) if you provided the information please leave a
note and let the referral department know via spark.
ONLY if further questions then transfer.

 
If Patient was in Urgent care/Emergency room for a matter related
with a referred specialist set a follow up appointment with PCP. (Ask
Patient to bring discharge paperwork to the appointment)

 
If Specialist office is calling to confirm patients information. (If
patient was not referred to that Specialist office they need to send a
release medical records form via fax)

When to transfer to referrals
If Dr´s office where the patient was referred is calling requesting
documents to be faxed such as:

° Labs
° Imaging
° Dr´s Notes

 
Imaging Centers calling to verify DX Codes for test.
 
Patient is requesting to change the assigned specialist (redirect).
 
Missed Call from Referrals Department. (See Notes in chart).
 
If the status of a referral shows “Pending” & patient is requesting to
know what's going on referral

 
If a patient needs an extension for a referral and is within 3- 4
months it has been requested

 
If patient is complaining of the service of the specialist and request
to be redirected to another doctor as long as its within 3 months.
